Identity documents: one for each person covered

A national ID, passport, driver’s licence or birth certificate for you and for each dependant, each in their own row. Add another file at any time: nothing you added before is replaced. PDF, JPG or PNG, up to 5 MB each. If you do not have them all now, you can still submit and add the rest in your applicant workspace. Your application can be approved without them, but a claim for a person is paid only once that person’s document is here. They are not sent to the payment-proof reader.Identity documents

These details and documents are stored in your fund workspace for the application review. Your phone number is used to sign in and to send you notices on WhatsApp or SMS; an email address, if you give one, can also sign in and receives notices by email. Hosting and encrypted backups are located outside Zimbabwe, and the fund sets the retention period — the privacy policy sets out both. Accepting these declarations records a request for review. It does not create cover, and no payment is taken.

What you’ll need

Your national ID, passport or birth registration number, and a copy of an identity document for each person covered (national ID, passport, driver’s licence or birth certificate)
A date of birth for everyone you add. It sets their adult or child rate
Your phone number (and an email address, if you have one), and your home address
The main member must be 18 or older. Cover starts on the 1st of a month: until the 7th you can start this month or next month, and from the 8th it starts next month
Your employer’s name and your employee or EC number, if an employer or the Salary Service Bureau pays
Any existing medical condition for you and each person you cover, with the medicine taken for it. A condition that is not declared is not covered

Choosing a package

Step 2, Your cover, shows each package, its overall limit and its indicative contribution. Open “Compare all packages” there to see them side by side. The fund confirms the contribution for everyone you include.
