You do not need an account or a PIN to send it. The fund reviews your application before cover starts. No payment is taken here.

A PDF of the whole form, or a clear photo of each page. PDF, JPG or PNG, up to 5 MB each, and up to 6 files. Adding a file never removes one you added before.

    Identity documents (optional)

    A copy of the national ID, passport, driver’s licence or birth certificate of each person the form covers, if you have them now. The fund asks for any that are missing.

      Your form and documents are stored in Budget Health’s fund workspace for review by authorised membership staff. Section 10 of the form is medical history: it is not shown to agents, and the payment-proof reader is not used for these files. How your information is used

      Hosting and encrypted backups are located outside Zimbabwe, and the fund sets the retention period — the privacy policy sets out both. Sending your form records a request for review. It does not create cover, and no payment is taken.
